Need Help Obtaining ESIC Form 7B Medical Acceptance Card for Employees?

You will find the ESIC declaration form itself. Please check the declaration form for the information you need.

Dear Shakuntala,

First, log in with the employer user ID and password. Then, click "Print Counter Foil," enter the insurance ID number of the employee to whom you want to issue Form 7B, and click "View." A page will open up. Click "View Counter Foil" towards the right side, and you will get his temporary identity certificate page. On that page, towards the upper left side, you will find a link for the Medical Acceptance Card [Click Here to View Medical Acceptance Card]. Click on it; this form is called Form 7B.
