Both the templates/formats are fine. Different kinds of leaves should have specific reasons to be mentioned. It would be better to indicate the following in the leave application:
- Leave address/contact numbers.
- The person to whom the charge is being handed over during the absence of the employee.
In senior/responsible positions, the incumbent should hand over charges to a designated person so that the flow of work during the absence of the employee/officer will continue.
It would also be better for the organization to issue an order stating the sanction of leave for the concerned employee/officer. Thus, a proper record of the leave can be maintained.
