Confused About EPF? How to Open a PF Account When Your Company Hasn't Provided One

I have an inquiry about EPF. I am working in a private company, and in my company, management is deducting PF without any PF account number, which is against the PF rule. Now they want to apply for an EPF number.

Could you please let me know the procedure to open a PF account, how to apply, and where to do so? Also, please provide information on the application form.
