Salary Slip Format In Saudi Arabia

Dear all,
Iam working as Hr Executive for a co in Saudi Arabia.My company plans to issue salary slips from this month.I need a format for it.Please help

In payslip, few components must be added like
Name
E.Code
Date of joining
Provident fund no(if applicable in SAUDI)
Account No.
Month
Total days
Payable Days
Leave allowed
LOP(loss of pay) along with these components you have to give the salary break up.
Right now i dont have any format with me, i will send it to you soon.

No need of salary slip............There is no need to pay even salary in KSA to run a business.........If any one strike management can easily put them in jail........
