Changing Date of Birth in EPF Records: What Forms Does an Employer Need?

This is regarding a change in the Date of Birth (DOB) in the Employees' Provident Fund (EPF). If there are any specific formats or forms that need to be filled out by the employer, please kindly suggest them to me.

Hi Ashwini,

You need to submit the cover letter with ID proof at the EPF office. They will then update their records accordingly.

What is the actual date of birth entered in the service particulars? On what basis? What is the DOB entered in the PF records, and on what basis? Either one must have proof. You decide which one is correct based on the proof. If the variance is in PF records, submit your proof to your employer and request to forward the same by submitting the revised Form V with a covering letter to the Regional EPFO for necessary changes. Any changes should be made on the prescribed forms only as prescribed under the Employees' Provident Fund Scheme, 1952.
