Guide for Changing Date of Birth in EPF: Forms and Process

Respected Sir/Madam,

I am the ESIC/EPF Incharge officer of my company. I have no idea how to submit a request letter to the EPF office for changing the date of birth. Can anyone please upload the EPF Date of Birth Changing form for me?

Steps to Change Date of Birth in EPF

Below are the steps to have the Date of Birth (DOB) changed in EPF. Also, find attached the form and the circular for the change process.

Steps of PF DOB Change:

1. Download the DOB correction form.
2. Fill in your details and print it.
3. Sign the printed form and get it signed by your employer.
4. Get it stamped with the company seal.
5. Attach the self-attested copy of identity proof with the application.
6. Now, it is the responsibility of your employer to send the application to the regional PF commissioner.
