Employee Engagement Activity Survey

Hi All
We, as HR professionals conduct lot of Employee Engagement Activities in our offices. These activities are in-office games, festival celebrations, etc. I need your help in designing a feedback form for these activities. The aim of the survey would be understand what type of activities the employees want us to conduct, do they enjoy these activities or not.
