I have been working at a firm for the past 4 years, and the key responsibilities assigned to me are entirely different from my designation. I have requested an offer letter with a designation that aligns with my responsibilities, but I have yet to receive it for the past 2 months.

Each company has the right to have its own designations for any role and responsibility. There is no law that defines the responsibilities associated with a designation. There is no point in taking this up as an issue. However, if the same organization gives you a higher designation for the responsibilities you handle, you can raise the issue during annual performance reviews if your performance is also seen as outstanding.
