Can any one please share ESI 7b form - couldn't find it on the ESI website

Can any one please share ESI 7b form-Urgently needed. I couldn't find it on the ESI website.

Hi Shravan,
Please follow below step to down load medical acceptance card.
- Please login your self to Employer portal
- Go to print counter foil (Middle section)
- Add the details of IP
- On left of page of that IP you will get TAB as Medical acceptance card in Blue.
- Click and take the print out.

Its nothing but MRE (Medical Record Envelope), a basic record to be maintained in respect of every Insured Person at their respective dispenasary.
When U for Print counter foil (i.e. Temporary Identity Card-TIC), at top u can get a blue link of form-7b....there u can get the print of a form 7B of the concerned Insured Person.
All HRs .. its better to have an habit, as soon as they register any Insured Peron, better to handover their concerned Temporay Identity Card and Form-7B by taking an acknowledgement from each of such Insured person.
