Company Tour- Employee Safety

Dear All Seniors,
We are planning for one day outdoor trip. Company owner ask to fill and sign the form by all employees which should consider the statements like "employees are responsible for own safety"
Can you please draft the form for the same.

I Mr/Mrs/Ms ----------- am voluntarily coming for the outdoor trip to------------ for enjoyment and entertainment.I fully understand that I am responsible for my own safety during the trip.I will abide by all safety rules while on trip.
My view:
Company cannot wash off its responsibility by such certificates.
Good arrangements for safety should be done and responsible people should accompany employees during the trip/picnic who will watch out for any potential danger to members/families etc..
