Form 10 C : Are You Availing Pension Under EPS-95 ?

I am stuck in a question while filling form 10 c "Are you availing pension under EPS -95 ?" Could anybody help me in this regard .

I don't know that why do you stuck for the 12th query of form 10C, "Are you availing pension under EPS -95 ?". Form 10 C is either for Scheme certificate or withdrawal benefit on our own contribution to EPS. But one can avail PF pension through the membership of spouse (widow/widower pension), parents (child pension) or any other member (nominee pension).

Sir , I don't know my PPO number and whom it was issued ? Is it necessary to mention it in that column

PPO is the abbreviation of pension pay order. This is applicable in case you are availing pension in any of the above categories. If not, the answer ''No" will do.
