Thank you for sharing the information. In fact, this form was uploaded on the EPFO website in the first week of November 2014, but with no instructions. Also, there was no order or circular up until now. EPFO officials were unaware of it. There was no clarification as to whether this new form is additional or replacing the old form/forms. Now, there is clarity in this regard.
