Of course, we understand your real name is Ankit S. Chauhan, whereas in the EPF account, it's given as "Ankit Singh." First of all, you have to confirm which is correct. As others have guided, first change your name in the PF account, then you should go for the claim. If you club both at the same time, you will land into a problem. Even if they send the payment, you'll have a problem while encashing through your bank account.
Service Records and Name Discrepancy
How about your service records? First, check whether your name is correctly recorded there. Why have you slept over the matter for so long? You are supposed to get your PF account statement every year, and any discrepancies should have been corrected then and there itself. How many years have you served in this organization? Did you change organizations anytime in the past? I'm surprised at how an employee could ignore a discrepancy in the name itself for so long!
