Where Can I Download Form 3A for PF Withdrawal? Seeking Guidance from Experienced HRs

Can anyone tell me where to download Form 3A? One of our ex-employees has submitted the PF withdrawal form, and the PF office has instructed us to submit Form 3A along with the withdrawal form to expedite the processing of the request. This is my first time coordinating the PF withdrawal for an employee, so I am seeking help from senior colleagues regarding the entire procedure. An early response would be greatly appreciated!

Now that you are submitting PF returns through ECR, there is no need to submit Form No. 3A.
