Amount Withdrawal Procedure From 6 Year Old Pf Account

Dear all,
Can you please help me to withdraw my pf money from my 6year old pf account. what is the procedure.
Company was HBL Global.

Dear Mr.Joby,
You will have to fill FORM NO 19 & 10 C and submit to the HR Dept /Person handling PF in HBL Global Ltd. along with a cancelled cheque.
It is suggested that the Forms should be complete and filled with the assistance of the HR person to avoid any cancellations / mistakes and bear a Revenue stamps and signature wherever required.
