Need Help Creating a Faculty Attrition Survey for My College Project – Any Suggestions?

I am new to CITE HR. I am an MBA student, and now I have to do a mini-project on faculty attrition at my college. My college is a self-financing engineering college in Kerala. Can anyone help me prepare a questionnaire for data collection? :)

Thank you for your reply. It gave me a lot of insights about the topic. However, I am actually searching for a questionnaire to conduct data collection on the attrition rate in self-financing colleges for faculties.
