Bombay Shop & Establishment License : Cancellation Procedure

Can anyone tell me the procedure to cancel a Shop&Establishment Licence.
Can you please list down the document required and what is the procedure.

Dear Robinson ji,
You need to just surrender your certificate of registration with covering letter giving reason and date of closure of your establishment at particular place.
