How to fill the PF monthly Challan and whats the meaning of A/c - 1, 2, 10, 21, 22?

Dear Seniors,
Kindly confirm if the attachment provided by Mr.Jai Bakshi is true and correct. As per me it has some flaws and loopholes. I badly need an explanation for filling up the combined challan. i am unable to understand as to how the figures will come in A/c 1,2,10,21,22 :( Please help.

If any employee did not come a single day in a month that per person is count in epf challan or not as a subscriber?

hii everybody, i want to know that PF challan should be filled by employer in seprate way of all employees or in combined way per month.
is there is necessary to fill the PF challan of each and every employees in seprate ways. please help me
