Requesting Letter To ESI Officer For Username And Password

Hi All,
I am working in a software company in HR dept.. recently we got applied for ESI for our employees and we submitted few documents and need to get the username and password from the ESI. Now we need to submit a requesting letter regarding username & password. So, can any one suggest me in this.

If you have submitted all the required documents they will mail you the coverage letter. You will get the login details in the coverage letter as well as by mail. I hope no need to send request letter for user name & password
