How Do I Draft a Request Letter to ESI for Employee Login Details?

Hi All, I am working in a software company in the HR department. Recently, we applied for ESI for our employees and submitted a few documents. Now, we need to obtain the username and password from the ESI. We are required to submit a request letter regarding the username and password. Can anyone suggest how to draft this letter?

If you have submitted all the required documents, they will mail you the coverage letter. You will receive the login details in the coverage letter as well as by mail. Hopefully, there is no need to send a request letter for the username and password.
