U can engage some consultant to conduct a paper - pencil survey, based on a questionnaire prepared & approved by your mgmt. Pls ensure that the objectives & deliverables are clearly informed to the agency to prepare relevant questionnaire based on your type of industry, location, org culture, etc.

The Q12 questionnaire can be a basis of any employee engagement survey. One can add more questions and tweak existing questions to suit your requirement.
U can do a trial survey on your own and then involve an outside consultant.

You can outsource this job to a Consultancy who do this kind of work. You can have paper and pencil survey or an online survey. In my opinion online survey is better as processing the results is much faster and it ensures a high level of confidentiality.
