Performance Aprisal Format For Trainers

Hi,
friends this is sunil, working as a asst. Managar- HR, for a company. we are in the corporate training provided to IT companies, we provides our in house IT trainer to the major it companies in across India. as a HR i need to prepare performance appraisal format for our trainers based on thier 1 year performance. so kindly help me how to prepare performance appraisal letter.

Dear Sunil,
You must be taking feedback at the end of the each training programme. Compilation of the feedback is more than sufficient to judge the performance of the trainer. In addition to this, you may award certain marks for development of new training programmes, suggestion of innovative ideas etc.
