2. Which form should be submitted to the ESIC dispensary or branch office?
3. How can he avail of this facility?

The employee covered under the ESI Act, 1948, who has to visit an outstation due to work relating to or in connection with the work of the factory, can obtain Form ESIC-105 from the employer and present the same to the nearest ESI dispensary where he is temporarily required to work. The blank Form ESIC-105 can be obtained from the nearest ESIC Branch Office.
