Seeking Your Input: How Can We Improve Our Post-Recruitment Feedback Process?

Could you please help me with a post-recruitment feedback form (the customers would be the employees who have recently joined our company)? This form would help in ascertaining the quality of the recruitment process - whether the recruitment process needs any improvement in terms of the time taken to reply back to the shortlisted candidate, or the attitude of the interviewer towards the candidate, or any other aspect that you feel is relevant to this context.
