Can we transfer PF from exempted to unexempted?

Yes, you can proceed by following the same process of filling out form no. 13.

Yes, you can transfer the PF of an employee from exempted (Trust) to unexempted (RPFC). You would be required to do so when an employee leaves the current company and joins a new company. However, in such a case, please advise the employee to also transfer the pension account from the current RPFC to the new RPFC.
