One Fatal accident happened at PGCIL (Power Grid Corporation of India Limited) (Contractor - Jyoti Power) Aurangabad Stringing work site, in which 4 workers died and 2-3 workers seriously got injured. This accident happens during final sag operation, in which dead end tower (nearby gantry) collapses and four workers, who was working on tower are crushed between collapsed tower.
Following are the probable causes of the accident.
(1) Back stay not given to Dead end tower.
(2) Over sagging which resulting collapse of tower.
(3) Final sag operation was carried out with two tractors.
(4) Winch machine usage was not in operation.
(5) Tower was not fully tightened.
Preventive Measures to stop this unsafe condition:-
(1) Back stay shall be fixed at least 1.5 times (height of tower) before start of stringing work.
(2) Depth of dead man pit should be 6 feet and minimum diameter of dead man wood should be 12 - 14 inch.
(3) Double back stay arrangement may be given to strengthen the back stay.
(4) Minimum 18 - 20 mm diameter Steel wire rope shall be used for back stay lead.
(5) Final checking of tower must be done before start of stringing activity.
(6) Use of winch machine.
Therefore please take necessary precautions during stringing work and suggest the same to team. This is for your kind information and necessary action please.
