I am currently pursuing a Master's degree in Personnel Management and Industrial Relations. My research topic pertains to performance appraisal for employees. I am in need of a questionnaire on performance appraisal for both employees and employers. Please reply to me as soon as possible.

Ramya, Have you searched CiteHR to see what has been posted on this topic? Also, kindly do a Google search. Have you done literature search and formed your aim and objectives for the project? Hi, A performance appraisal form should cover the following 3 aspects of a role:
A) Performance.
B) Potential.
C) Soft skills/attributes required in the role.

The form comprises of 3 parts. Initially, the form is filled out by an employee. Then, the employee is rated on the same parameters by his manager/supervisor. Finally, in the third part, the ratings are discussed among the manager, the functional head, and the business HR.
