Coaching and Sharing - Key strengths and areas of improvement
Evaluation Ratings and Rationale - Agreement and sign-off
Conclusion - Review planned goals and schedule for next review

hi,
if the organization is big like 2000-3000 employees then u should go for a customized software developed for the HR eg (HRIS & HRMS etc).
It would be more feasible and easy to use for the entire organization.
