Some employers may require this form to be certified by the Head of the Institution or respected individuals such as doctors, bank managers, postmasters, etc. You may want to consider introducing a character verification certificate along these lines.
2. **Medical Certificate**: It is also important to have a medical certificate at the time of joining, especially for technical posts, to ensure that the selected candidate is suitable for appointment. This is particularly relevant for roles in electrical, chemical industries, drivers, etc.
3. **Required Documents**: These include a Bank Account Opening Form, Nomination forms for EPF and Gratuity, passport photos, and family photos for IP cards for ESI membership.
4. **Address and ID Proofs**: These are essential.
5. **Experience/Relieving Letter**: A copy from the previous employer is necessary.
