How To Avail PF Pension Benefit

Please let me know that after completing 10 years of service how to avail the PF pension benefit.
How the amount of pension calculated, if possible an illustration may please be provided.

Kindly provide one example vide Date of Birth, Date of Join, Average salary of last twelve months etc.

Average Salary (Basic+DA) of last one year-14500
