Go to epfindia.com and you will find the link for downloading the NSSN form. It is in PDF form. Also there is guideline for filling it and further submission.

NSSN form is available in PF dept website, but this is for only information purpose. If you want to submit the NSSN for then you collect original with serial number printed forms from PF dept.
