Auditor wanted safety records - not able to show record for "high risk area"

I am working in Gujarat. Couple of days before, our company faced an audit. The auditor wanted me to show all safety records, but I could not show the record for the "high-risk area." I have searched on the internet as well, but that didn't provide me with good results. Is there any format available, or do I need to create one? Please help me.

High-risk areas are identified through Risk Assessment, HIRA (Hazard Analysis Risk Assessment), as per the criteria defined in your procedure manual. If it is not documented in the work area, you cannot demonstrate the presence of a high-risk area to the auditor. Please refer to your procedure manual.

You can find the format on how to make a safety audit report on the Internet. It includes all the things that have been checked, executed, and what contingency plans you have in order to escape from the high-risk areas if any mishap occurs.

Risk assessment depends on the industry's process. The risk assessment to be conducted from the main gate to the rear, including peripherals. Each floor/process needs to be assessed, and now it has become mandatory after the major accidents that happened in Bangladesh and Pakistan. The Social Accountability International (SAI) has introduced a new amendment on risk assessment, so you can access the document along with the format from their website.
