Seeking Guidance on Annual Report Submission under Employee Compensation Act

Till now, I have been helping members clear their doubts about labor laws, but I have come across a situation where I seek the help of my seniors. I was asked by some of my acquaintances to help send the Annual Report regarding a factory located in the state of Haryana under the Employee Compensation Act. However, I am sorry to say that I could not find any mention of the Annual Return in the state rules. I am aware that the State of Haryana has adopted most of the rules of Punjab upon its reorganization, but the same is also not found in the Punjab rules.

Although I have been able to locate the Annual Return in Master Form in Form 21, which resembles the Annual Return under the Factory Act Rules, it requires particulars on all other Acts, including the Factory Act (copy attached). I seek help to confirm whether it is the same or if a separate return is prescribed for the state of Haryana. The expert advice of the seniors is solicited.

I posted an email seeking clarification on the Employee Compensation Act - Annual Report on December 07, 2013, but so far, nobody has responded. I once again request the members to provide their feedback as soon as possible. The matter is urgent.

Employee's Compensation Act, 1923 read with the Workmen's Compensation Rules, 1924. If there is any separate annual return for Karnataka, please share the format.
