Urgent Help Needed: Which Forms Are Required for Labour License Renewal in Chennai?

Kindly let me know which form has to be submitted to the Labour Office for renewing the Labour License in Chennai. My staff member, who is in Chennai for the license renewal, has informed me that he needs to submit a specific form along with the application.

He mentioned that Form VI or Form VII should be attached along with the application. Please advise if any additional forms are required in conjunction with Form VI and VII.

I would appreciate your assistance in providing the necessary form for the license renewal process. This matter is urgent.

Attaching the form for your reference. Form VI is for a new application, and Form VII is for renewal. The following are the required documents for renewal:

1. Form VII
2. Copy of Work Order/Agreement
3. Proof of the status of the contractor's firm
4. Demand Draft

These documents need to be submitted thirty days before the expiry date of the license.
