For JD Matrix, you can include the following things:-
Name of the position
Immediate Reporting head
Department
Experience required
Qualification required
Softskill required
Skills desired
Detailed KRA
you can prepare it in both word or excel

You JD focus on your position requirements like,
1. Qualification of Technical skills, basic educational, etc.,
2. Communication skills
3. Works to be carried out - Daily, weekly, bi-monthly, monthly etc.,
4. The responsibiolities
5. Reporting system
6. Focus area of work. etc.,
based on that you have to formulate the matrix with expected outcome in metrics.
The expected and landed outcome to be measured in the PMS.
