The form is good; however, you can add some more points for clarification and future analysis, such as:
1. Is this a new position or a backfill position?
2. Can anybody from the department/organization be promoted or transferred to do the job?
3. Whom will the person report to/reporting structure?
4. If this is a contractual position, then for how many months/years will the contract be?
5. Soft skills required for the position.
