What course are you pursuing and where will you be using the questionnaire? Have you done any literature survey and decided on the aim and objectives of your study?

Dear Sonam As far as I guess you want questionnaire for your BBA or MBA thesis. So, your questionnaire should be consist of minimum 20 question. It is better to include all close ended questions because they are easy to analyze. Questions should cover all the factors that can increase or decrease employee motivation. for e.g salary packages, fringe benefits Company HR policy for allowing leave, grievances handling, supervisor behavior etc
