Dear Sonam, I assume you need a questionnaire for your BBA or MBA thesis. Your questionnaire should consist of a minimum of 20 questions. It is better to include all closed-ended questions as they are easier to analyze. The questions should cover all factors that can increase or decrease employee motivation, such as salary packages, fringe benefits, company HR policies for leave allowance, grievance handling, supervisor behavior, etc.

Are you conducting any research on employees? It would be better to handle the situation through behavioral training for all levels (Operator, Supervisor, and Executives) along with psychometric assessment.
