It is very easy to fill out this form:

1. Full Name
2. Father's Name
3. Company Address: (Leave Blank)
4. PF Number: If you know, please write it; otherwise, leave blank
5. Date of Leaving: Suggested to leave blank
6. Reason for Leaving: Why?
7. Full Name: Write your full name again
8. Father's Name: ===="====
9. Address: Your complete residential address
10. Bank Account Number: _________
11. Bank Name, Branch & Complete Postal Address of the Bank

Sign and submit to your company. It's that simple. If you still encounter any issues, please contact me at 9820592842.

Dear,

Yes, you will have to sign that column if you want to withdraw the PF. It is compulsory for all. It is just a formality from the PF department.
