We are paying incentives other than salary based on monthly performance. Do we need to pay ESIC on such incentive payments?

If it is monthly and shown on the salary slip, then yes, you need to pay ESIC on such incentive payments. However, if it is shown on the pay slip but paid at intervals longer than two months, then no, ESIC is not required.
