Document Needed Maternity Leave

Dear all,
I currently working in Software company as a post of HR executive. In our company one female employee want to maternity leave and she has be submitted to leave form with EED.
But my problem is that what are the documents to be needed? Any doctor's certificate is needed?
Please help me..

Is she covered under ESI or not,pl give details .
1 .Her date of joining.
2. How many days she worked in your CO as on today?
3..Her date of expected confinement.

As she is not covered / or entitled under ESI scheme she should be given MB under Maternity Benefit Act1961.
1-She is entitled for the benefit only if she had worked with you for at least 80 days in the period of 12 months immediately before the expected date of confinement.
2-If so ,she is entitled for MB @ average wages for 6 weeks before and 6 weeks after the delivery.In addition she is entitled for benefits @same rate for 30 days if she is suffering from any illness arising out of delivery.
3- She is entitled for absence from work during the above period.
4-If you are not providing any pre or post natal care free of cost you have pay her Rs 3500/ as medical bonus.9
5-You may ask for medical certificate in Form B or any other form prescribed by your state govt:

And one another query is that she want to one month leave after 6 weeks of maternity leave period. So is it possible?
If possible that this one month leave is payable?

Yes she can, after post- natal leave.

It is learnt that the MB entitlement has gone upto 180 days.

180 days is in govt service .Not as per Maternity Benefit Act 1961.
