Payslip Format - Salary Slip

Hi,
Can anyone please help me out with the Payslip format for a start up company.
I need a format where there is only tax deductions should be showed and there will be no other deductions in the salary.
So please help me out with this kind of salary slip or payslip.

Please let me know the format with out any HRA or any other deductions in salary except the TAX part in payslip.
And if possible please let me know how much will be the TAX deduction for an employee.
