Could you please send me the guidelines/instructions to fill Form APPLICATION FOR ISSUE OF DUPLICATE STATEMENT OF MARKS.

Brief: I completed my graduation from a regular college under D.U. One of my exams was not cleared (IIIrd Year). Afterwards, I reappeared for the exam and passed. After some time, I received a mark sheet showing one exam was not cleared and a provisional certificate from my college. Both documents have different roll numbers.

So please instruct me on which roll number I have to specify: the Provisional certificate roll number or the Mark-sheet roll number.
