I Need Warning Letter For Employee For Illresponsibilty

i need letter for employee he is using company car his tyre punchered but telling admin persons to arrange so i want to tell him this is not company responsibilty this is his own responsibilty so how i can wright this letter please advise me quick help will be appriciated thanks
