Forms In Haryana Shop & Establishment Act

Dear all,
Please help me with the Various form available in Haryana Shop & establishment Act.
Recently i would like to know, which form is needed to submit for the employees who is left company.

Dear Member,
We are required to submit Form “G” to the Shops Inspector of the area regarding the employees who have left the job as per requirement of section 13(4) of the Punjab Shops & Establishments Act, 1958. For having knowledge on various forms you may go through Punjab Shops & Establishments Rules, 1958.
