How Can I Write an Email to Get My Relieving Letter After Leaving Early for Medical Reasons?

I was working with the organization. Due to medical issues, I could not complete my 30 days' notice period. I had completed 27 days. However, I had to leave the company early for medical treatment. My manager informed me that he would not provide the relieving letter but mentioned that he would accept an email from me for the relieving process.

I am requesting a format for an acceptance email for my resignation. I left the company two months ago. Please provide me with some suggestions.

A relieving letter is to be issued from the company, and I doubt if one written by you will hold any importance when you are job hunting (even if it is accepted by your employer).

If your company allows, you can ask to adjust your pending leaves against the 3 days that you were unable to serve. Also, mention to the manager that you couldn't come due to medical reasons and that this, being a genuine case, can be excused. Apart from that, also suggest that you would be okay if the pay for these 3 days were deducted in your F&F.

However, as far as my knowledge goes, a relieving letter ought to be from the company's side and that too on the company's official letterhead.
