Delay in confirmation of some employees - While issuing confirmation for such employees, what steps should HR take care of?

Dear all ,
I would like to go throught My standard Resignation for not satisfying replay from my HR team.
So I am requested you to please send a format copy of resignation to me that i am submit to HR team.
Here by please send a resignation too hard that managment may know where is mistake happen.please use a professional hard resignation latter to me.

Thanks to all for help me in my issue , Finally I am conformed with 8% raisel in CTC.
But Still I have a One dount related my Carrier That Can I am eligible in next Appraisel OR any Special term in Appraisel than other Employees.
