Interviewee Assesment Sheet - Doc Download

Dear All,

Can somebody please email the format of the interviewee assessment sheet that is filled once the interview with the candidate is over, and the interviewer has to leave his/her remarks about the candidate in an easy-to-understand manner? This will help ensure there is no scribbling at the back of the CV, which is usually done by the person conducting the interview.
