Understanding Appraisal Calculations: Is It Based on Gross Salary, CTC, or Basic Pay?

I have one doubt: is appraisal calculated on gross salary, CTC, or basic? Can anyone provide me with an appraisal format, steps, and calculations? Please revert as soon as possible.

Appraisals will happen on the CTC part only. To prepare for the appraisal, you should get the appraisal forms filled by yourself and the appraiser of the concerned department. Based on the various parameters, you should refer to the management, and they will make the decision on that.
