Please help me in finding answers to the following queries:

01. Various forms and purposes of the forms.
02. If an employee is injured, can he join any private hospital without enrolling in ESI?
03. What is the procedure for claiming?

Dear,

1. Form - 2 is for registration.
2. Form - 37 is for certifying continuous employment.
3. It is the employer's responsibility to ensure that everybody can get an ESI card on time.
4. If anybody has an ESI card (Temporary/Permanent), they are not entitled to receive any benefits from a private hospital unless referred by the AMO in a complex case. The employee must be admitted to an ESI hospital in case of any sickness.
5. Whether going to or coming from the office, it is the employee's responsibility. If they have an ESI card, they will receive the benefits.
