01. Various forms and purpose of the forms
02. If any employee injured, can he join in any private hospital without join at ESI
03. WHAT IS THE PROCESURE FOR CLAIMING
04. IF ANY EMPLOYEE INJURED WHILE COMING OR GOING FROM OFFICE

Dear,
1. Form - 2 for registration.
2. Form - 37 for certifying continuous employment.
3. It is employer responsibility to ensure that everybody can get ESI card in time.
4. If anybody has ESI card (Temporary/Permanent), he is not entitled to get any benefits from Private hospital unless AMO refers on any complex case. Employee must admit into ESI hospital in case of any sickness.
5. Going from office or coming to office it is reponsibility of the employee. If he has ESI card, he will get the benefits.
