Seeking Feedback: How Can I Improve as a Team Leader Before I Resign?

Hi Folks, I am in need of an evaluation form to gather feedback from my team members before leaving my current organization. I have a small team of 25 members and am planning to resign. It would be helpful if someone could share a team leader evaluation form or provide a questionnaire on the same. Understanding the areas where I need to improve is crucial for me. 

I have attached a form and hope you find it useful. You can modify it as per your needs. Since you have not provided your profession, that aspect has not been covered in the questionnaire. General behavioral aspects have been included. The form can be modified and used with two rows: one for self-assessment and the other by a superior or peer.

There are plenty of 360-degree appraisal forms available, and you can find them by browsing Cite HR Archives or the internet.
