Please tell me the use of Form No.7B of ESIC as one of our employee who covered under ESIC went to ESI hospital for availing medical treatment but doctor asked him to take form no.7b from your organisation...

Form 7B of ESI is a medical acceptance card. This card contains the application by IP to include his name in the list of Doctor and the acceptance by Doctor.
This card is available on registering IP online.

please send for 7B (Medical Acceptance Card) PDF Format to my mail id. Our ESI Dispencary staff is ask to bring from our organisation only

Medical Acceptance card (7b) is available in ESIC website. you can log in with your credentials in to site then print counter foil tab, you should provide IP Insurance No and submit then will appear left site medical acceptance card format with pre printed IP details. you can download yourself and give to employee.

For Medical Benefits (CASH LESS) under ESIC employee need to submit under mentioned documents to dispensary -
1. ESIC Card's xerox attested by employer.
2. Temporary Identity Certificate/ E-Pehchan Card with Aadhar card number attested by employer.
3. Entitlement to Benefit Certificate through esic.in/employeeportal/login.aspx
4. Eligibility Certificate by employer.
